Morpheus instructions for use

These instructions describe the correct use of Morpheus. Read them carefully and follow them. The Morpheus support team is available during normal business hours.

Diagnosis, medical or therapeutic recommendations, and treatment are solely the responsibility of the treating therapist; they are not provided by the software.

Intended purpose

  • Morpheus is an app that supports psychotherapy.
  • It supports self-management in imaginal (in-sensu) interventions during cognitive behavioural therapy.
  • It is a blended-care mobile application for digital psychotherapy.
  • Patients can record imaginal exposure during therapy sessions on their smartphone or tablet.
  • Patients can listen to recorded therapy sessions at home. Repeated listening between sessions is part of standard imaginal interventions, referred to here as between-session self-management in-sensu exposure (BSSSE).
  • Morpheus supports monitoring stress during BSSSE by repeatedly asking for a stress self-assessment.
  • It helps therapists and patients monitor progress within sessions and overall through an overview of pre- and post-evaluations.
  • Under standard conditions—listening to sessions without Morpheus—high-stress situations often occur and weaken therapy effectiveness.
  • Morpheus helps prevent this by repeatedly asking for stress self-assessments and stopping audio playback when stress exceeds 70 on a scale from 0 to 100.
  • When stress exceeds 70, Morpheus reminds the patient to apply a stress-regulation skill.
  • Morpheus provides digital stress-regulation games (distraction) when stress is too high.
  • These are interactive mini-games addressing different sensory modalities, such as auditory, motor, visual, or logical tasks.

Medical indications

  • Morpheus should be used by patients undergoing behavioural therapy with an imaginal self-management module.
  • It should be used when the therapist integrates it into therapy and explicitly recommends its use.

Diagnoses for which Morpheus should be used

ICD-10:

  • F40: Phobic anxiety disorders
  • F41: Other anxiety disorders
  • F42: Obsessive-compulsive disorder
  • F43: Reaction to severe stress and adjustment disorders
  • F44: Dissociative (conversion) disorders
  • F45: Somatoform disorders
  • F48: Other neurotic disorders
  • F60: Specific personality disorders

Medical contraindications

Morpheus must not be used in group therapy; it is for individual therapy only.

Contraindications, ICD-10:

  • G40: Epilepsy
  • F00: Dementia in Alzheimer disease

Application

  • Use Morpheus during therapist-guided imaginal exposure sessions to record the session.
  • During self-management, listen to the recordings made during those sessions.
  • Use digital stress-regulation skills during self-management to regulate stress.
  • Use the statistical overviews during therapist-guided exposure sessions to monitor progress.
  • Delete Morpheus from the smartphone after therapy ends.
  • Patients should try every digital stress-regulation skill at least once before the first exposure session.
  • Discuss questions about the statistics thoroughly with the therapist.
  • Therapists and patients should initially adapt the self-assessment interval to the patient's risk of dissociation. The interval can be adjusted throughout therapy.
  • Using Morpheus is optional, not compulsory.
  • Patients can add individual text-based skills. Review these with the therapist before use.
  • Morpheus may give a false sense of security. Patients and therapists should therefore pay particular attention to external risks.

Setting of use

Morpheus is installed on the patient's smartphone and used in a formal psychotherapy setting together with the treating psychotherapist.

During an individual therapy session, patient and therapist use it to record the session. Patients take the recording with them, for example to their home or inpatient room, and listen daily until the next appointment as instructed by the therapist.

During therapy sessions, therapist and patient jointly evaluate the recorded self-assessments (statistics).

Morpheus is updated regularly to address security vulnerabilities and other problems. No substantial changes to functionality or design are planned in the near future unless risks arise from them.

Age limits

Morpheus should only be used by patients aged 15–65, in consultation with their therapist.

How it is used

  • Everyone participating in the therapy must consent to the use of Morpheus.
  • Install the app on the patient's smartphone or tablet after the treating therapist has instructed its use.
  • Morpheus can be downloaded from Google Play and the Apple App Store.
  • Use Morpheus from the beginning to the end of therapy.

Type and duration of use

Morpheus is downloaded from Google Play or the Apple App Store. Each access key provides 90 calendar days of use after initial registration.

Technical requirements

A smartphone or tablet capable of recording and playing audio is required. Morpheus runs on the usual iOS and Android operating systems.

Recordings can be played through external speakers or headphones. An external microphone can improve recording quality if needed.
